Responsibilities
- Works with operational Home Health & Hospice leaders (Intake, Scheduling, Quality , Nursing) to identify challenges and opportunities within their areas.
- Catalog existing reporting from Epic, Qlikview and SHP.
- Understanding of departments and initiatives leading to improved network integrity.
- Maintain dashboards for each area to provide insight and support analytics needs.
- Uses quantitative and qualitative methods to systematically collect and analyze data to collect and analyze data to inform daily operations and long-term strategy.
- Assist in the creation of a summary template for Home Health and Hospice to collect and present data on referral volume, conversion and quality outcomes.
- Review trends and opportunities to assist leadership.
- Submit data summary to internal and external referral sources on routine basis.
- Provide support to other Integrated Care Management leaders as needed.
- Creates an understanding of each ICM department to be able to assist with recommendations to existing reporting structures.
Requirements
- Required Experience: 2-5 years related experience required.
- Ideal candidate possesses a combination skilled analytics and knowledge of the healthcare industry.
- Experience within Virtua preferred.
- Experienced with patient tracking and service utilization programs.
- Proficient in Excel, Word and PowerPoint
- Required Education: Bachelor’s degree required.
